[-empyre-] skin deep or lower?

I'm interested in mods but i wonder how different they can be from the
original the engine was designed for, in certain ways? I have never made a
mod, so I'm not sure. Is the art of mod making primarily graphical and
sonic? Is there much programming? I imagine that depends on the extent of
the programming of the 'play mode' of the original engine. Is a mod just a
skin change?
